Patient's that are in our sync system that have had a care plan completed.

Patient has been entered into the sync system to be enrolled in the program.

Patient refused CCM or no longer wants to participate
4.
Patient's who have not answered in 3+ consecutive months or have a wrong number listed (supervisor approval needed)
5.
Patient's that do not have an adequate amount of chronic conditions, hospital patients only, dialysis patients with nephrology clinics, nursing home/rehab patients
6.
Patient's who's insurance does not cover CCM.
7.
Patients who are no longer living
